Sildenafil Citrate (aurogra) Action Mechanism
Upon sexual arousal, nitrogen oxide (NO) is released in the penis in the cavernous body. The released nitrogen oxide helps in the activation of an enzyme called guanylate cyclase which in turn causes the increase of another enzyme called cyclic GMP or cGMP.
The elevation of cGMP levels will help in their interaction with proteins present in the smooth muscles of the penis in the cavernous body. This interaction will lead to relaxation of the smooth muscles in the penis thus enabling more blood to enter the penis resulting in erection.
Sildenafil citrate works by being an inhibitor of another enzyme called PDE5 or phoshodiesterase type-5. The PDE5 is an enzyme that is responsible for reversing the erection process by causing the breakdown of cGMP. The metabolism of Sildenafil citrate may be affected in the presence of cytochrome P450, isoforms 2C9 and inhibitors 3A4. Isoenzymes of P450 also might lead to reduced metabolism of the medication. The concomitant use of medication like cimetidine, ketoconazole and erythromycin also affects the metabolism of sildenafil and concomitant use is not advisable.
The bioavailability of active constituents of the medication is not altered when antacids are taken concomitantly. The effects of Sildenafil citrate is not altered in the presence of ACE inhibitors, beta-blockers, calcium antagonists, diuretics, rifampicin, barbiturates, warfarin, tricyclic antidepressants, and tolbutamide.
When used with aspirin, there is no alteration to the bleeding time. There is no significant change to the antihypertensive changes alcohol induces when Sildenafil citrate is used in combination.
Amlodipine can cause adverse reactions when taken concomitantly with the medication. Sildenafil citrate is also known to reduce blood pressure when it is taken in combination with nitrates and donors of nitric oxide and should be therefore avoided.
The effects of a combined use of Sildenafil citrate with other medications available for erectile dysfunction have not been thoroughly studied. Hence, the drugs should not be used concomitantly.
